We adapted the CONSISE household transmission study protocol for use in South Africa and found it incredibly helpful to have the template. Some lessons learned:

Cannot predict how severe the flu season will be, so may be difficult to reach sample size.
When to stop—annual flu epidemic? Flu season may have a long tail, but will need to test many patients with respiratory illness to find few cases of influenza
Complex study design
Expensive--needs lots of staff and testing
We used a rapid test which had low sensitivity and poor PPV (and difficult to procure in South Africa)
Difficult to enroll patients (don’t seek care early in illness)
Difficult to enroll family members (May work outside home, and don’t consent to be enrolled, since index case may not be decision maker)
